New Patient Registration

All new patients will be asked to complete a short questionnaire if you are unable to complete you will be asked make an appointment for a registration medical. This will consist of an appointment with the practice nurse who will record your height, weight, blood pressure and test your urine. 

Patient's medical notes can take several weeks to be transferred to the new doctor therefore this registration completion/medical is most important as it enables us to give the best patient care in the meantime.  Failure to attend for a registration medical if required may jeopardise one's entitlement to register at this practice.
